In the ep, dean is stuck in his own mind/dream. He snaps his fingers, thinking he has control and can get out. He can't and he's left, essentially, facing himself.
What follows is really hot and really scary as Dream!Dean confront Dean about all of his insecurities. It's really wonderful acting from Jensen.
